The New Jersey Choose Life license plate is an official organizational plate administered through the Children First Foundation. Every plate serves as a visible reminder of hope while helping support organizations that provide life-affirming care and resources.

How to Order

  • Become a Member
    Join The Children First Foundation with an annual, tax-deductible $25 membership.

    Become a Member

  • Complete the Application
    Download and complete the New Jersey license plate application, then mail it to The Children First Foundation. We’ll co-sign the application and forward it to the New Jersey Motor Vehicle Commission (MVC).

    Download Application

  • Include the MVC Fee
    Include a $25 check payable to the New Jersey MVC with your application. This is the one-time plate switching fee.
  • Receive Your Plates
    The New Jersey MVC will mail your new Choose Life license plate directly to you.
