Get Your Connecticut Choose Life License Plate

Help put hope in motion across Connecticut.

Connecticut became the first state in the Northeast to approve a Choose Life license plate, helping bring greater awareness and support to organizations serving women, children, and families throughout the state.

How to Order

  • Become a Member
    Join The Children First Foundation with an annual, tax-deductible $25 membership.

    Become a Member

  • Complete the Application
    Download and complete the Connecticut license plate application, then mail it to The Children First Foundation. We’ll co-sign the application and forward it to the Connecticut Department of Motor Vehicles (DMV).
  • Include the MVC Fee
    Include a $65 check payable to the Connecticut DMV with your signed application. This is the one-time plate switching fee.
  • Receive Your Plates
    The Connecticut DMV will mail your new Choose Life license plate directly to you.
